TRANSFUSION CONFUSION! I was recently introduced to these scary blood transfusion reactions with adorable acronyms. Ive now seen hemoglobins dip below 7 and liver folks exsanguinate - so ive put in my share of blood transfusion orders and never truly worried much about reactions. My co rezzy gave a nice little chat on rounds introducing me to TACO or Transfusion Associated Circulatory Overload where the addition of this extra volume causes signs of overload such as pulm edema, increase in BNP, as well as tachycardia, hypertension, JVD. Its bad. Diuresing the patient is usually a good solution. TRALI, on the other hand, is similar in that respiratory problems are manifested as well however, TRALI is more associated with hypotension and has much less of a response to diuretics. Everything we do in medicine has an effect- both positive and negative and no matter what we give, IV fluids or opioids, we must balance the desired effects with side effects. Sometimes, the treatment is worse than the disease and its in those moments i like to remind myself: first, do no harm. ๐ŸŒฎ๐Ÿšƒ๐ŸŒฎ๐Ÿšƒ๐ŸŒฎ#tacos #taco #trali #trolley #donoharm
